Health Questionnaire and Food Diary

Before your first assessment, you will be asked to complete a detailed health questionnaire and a 3-day food and symptom diary. These provide essential insight into your current health and help identify underlying issues.

If you have any accessibility needs or would prefer, you can choose to complete these during your initial consultation instead. Other forms are also required for completion such as

Introductory Appointment – £100 (60 minutes)

This appointment is not a substitute for a full nutritional or dietetic assessment but can be a useful way to start if you are unsure or do not require a more in-depth assessment. If you are unsure whether a full dietetic assessment is right for you, and would like to speak with a Dietitian Nutritional Therapist to talk through your health concerns, the introductory appointment provides a first step. It offers the opportunity to discuss your main concerns, reflect on your goals, and consider your next steps.

Most people will then go on to choose a full Initial Assessment or one of the Bundles, as this provides the most comprehensive way to move forward. However, if cost, accessibility, or other barriers make this difficult, I offer a concessionary option: completing a health questionnaire between sessions and booking a shorter 30-minute Initial Assessment top-up (£80) to complete your assessment and plan. This option is available where it is genuinely needed to reduce barriers and support access to healthcare.

Initial Assessment Appointment – £180 (60–90 minutes)

Your initial assessment is an in-depth consultation where we review your health, nutrition, and medical history. This is invaluable in identifying potential root causes of symptoms and understanding unresolved or ongoing health issues.

The information gathered allows me to design a personalised nutrition and lifestyle plan tailored to your needs.​​

Follow-ups are for ongoing support to review progress, adjust your personalised plan, interpret test results and review supplementation where appropriate.

Supplements & Testing

Supplementation, medical and/or functional testing recommendations are made if required. Prices are set by the supplementation and testing companies and are an additional cost.
